1. Type or print the entire application. (Click here for a printable .pdf version of the application. If you do not have Adobe Acrobat Reader installed on your machine, click here for a free download.)
2. Selection of pieces for the exhibition will be chosen from 35mm, cardboard or plastic mounted slides. No glass mounted slides will be accepted. Artist may also submit images on computer Compact Disc (CD).
3. Each artist is encouraged to enter as many pieces as desired. The entry fee is $20 for up to three entries, $5 for each additional entry. Three-dimensional pieces may have up to three slides submitted showing different views of a same piece at the single slide rate. This also includes CD entries.
4. Label each slide as per diagram. PLEASE TYPE OR PRINT CLEARLY.
5. Enclose in a large envelope the completed application form, slide/CD fees, slide/CD holders, and a SELF-ADDRESSED, STAMPED BUSINESS-SIZED ENVELOPE for the return of the slides/CD. Slides/CD of accepted works will be returned at end of the exhibition.

ARTIST'S CHECKLIST - Have you:

• Completed all parts of the entry form?
• Enclosed a check payable to Art With a Southern Drawl for slide fees?
• Enclosed a self-addressed, stamped envelope for the return of the slides/CD?
